Grant Application – Criteria

Please carefully read the criteria. Unless the board agrees otherwise, applications submitted that do not meet the criteria will not be considered. The Foundation may also request additional information from you after the application is submitted.

In evaluating applications, the Board will consider applications based on the following criteria:

  1.    The applicant must be parents or legal guardians of children 21 years old or younger, or adults who live in the United States and receive and pay for medical procedures, treatment, and ongoing care.
  2.    Awards are limited to a medical condition that is considered to be serious and life-threatening in nature and, as a result of the procedure, treatment and care required imposes a serious financial burden on the family.
  3.    An application must be submitted in writing or send your application via email to the Foundation Board of Directors. The applications should be in the form of a letter outlining the situation, and in some cases, the Board may ask for documentation verifying the financial hardship, including copies of supporting material including W-2’s, recent pay stubs and tax returns, insurance limits.
  4.    In order to apply for your child, the child must live with you 51% or more and be listed as a dependent on your most recently filed IRS 1040. If the child is not listed on your most recently filed IRS 1040, then we need a copy of both your most recently filed IRS 1040 and the most recently filed IRS 1040 on which the child is listed as a dependent.
  5.    Applications may be reviewed by a health care professional appointed by the Foundation to determine if the procedure, treatment or care meets accepted medical standards and is appropriate for the condition of the patient.
  6.    The amount awarded to an individual within a 12-month period is limited to 85% of the fund balance.
  7.    Awards to any one individual are limited to a lifetime maximum of $1,000,000.00 unless the Board, for good cause shown, votes to exceed the maximum.
  8.   Unless the Board agrees otherwise, applicants who are not approved by the Board must wait a period of twelve months before re-applying, unless the medical condition and requested items have significantly changed from the original request.
  9.   The Foundation does not discriminate against any applicant because of race, color, creed, religion, sex, sexual orientation, disability, marital status, ancestry, or national origin.
